Important Notice to our Members – the 2011 Conference

As part of a more integrated approach towards Shifting the Balance of Care the Scottish Association of Community Hospitals has been invited to support a joint conference this year with the Scottish Government’s Reshaping Care Improvement Network.

The Reshaping Care Improvement Network aims to:-
‘Drive progress in Reshaping Care through shared learning, spreading good practice and increasing the pace of change’.

A number of organisations are also supporting the conference in collaboration with the Scottish Government and the Joint Improvement Team, including the Community Health Partnership Association, and relevant professional bodies.

The 1 day Conference “Reshaping Care-Improving Community Practice “ will be held on the 28th October 2011 at the Macdonald Inchyra Grange Hotel Polmont Falkirk.

Objectives for the full day practice exchange on October 28th are

  • To engage a wide range of primary, community and secondary care practitioners in Reshaping Care for Older People
  • To exchange practice in collaborative co-ordinated community care with a particular focus on clinical care for frail older people at home
  • To share experience in managing transitions of care and in providing comprehensive care out of hours
  • To promote closer working between the extended primary care team, community hospitals, secondary care specialists and mental health services
  • To explore key enablers for improving quality, efficiency and integration

The Scottish Government will be inviting NHS Boards to nominate up to 10 people to attend the event and a draft programme will be available from the Scottish Government JIT team on the 8th August 2011.

NB -There will also be a 1 day event on Thursday 27th October 2011 on Delayed Discharge and Intermediate Care at the Inchyra Hotel Polmont.

This event is aimed at staff with responsibility for discharge of patients and intermediate care and members of SACH are welcome to attend both days.
